1. At Compressor Stations: Review blow down,
excavation procedures, and policy re: wearing of fire retardant
coveralls.

2. When using scaffolding around valves, test heads
or any other location, make sure there are adjustable screw jack
type feet which are set on a solid
footing.
|
|
|
|
3. Cutting and pulling fill line over roller - it
may twist and turn. Be cautious, don't turn your back on it. Don't
get caught between the pipe and welding rig. Park the welding rig
away from the pipe.
|
|
|
|
4. Use check valves on test head end of compressor
hose in case compressor hose blows - check valves prevents full pipe
pressure backing into hose and subsequent violent flogging of hose
which can kill or seriously injure
workers.
|
|
|
|
5. Pre-test squeeze iron (iron from squeeze pump to
test head) to 25%- 50% over test pressure. When using hose it should
be certified. Look for tag identifying tested pressure.
|
|
|
|
6. When squeezing ensure only authorized personnel
are allowed on site.
|
|
|
|
7. When starting work at a test point and moving
pipe apart for pigging and testing procedures, pipe can be sprung -
Caution where positioning men - avoid injuries.
|
|
|
|
8. Do not use a 90 degree nipple for blowing down -
pressure can twist it off and turn it into a lethal projectile. 45's
are better, but the safest blow-down is straight up.
|
|
|
|
9. Clear the area in front of the trap when running
the construction pig.
|
|
|
|
10. Always use a bolted door. Don't use doors
equipped with pins.
|
|
|
|
11. Do not use slotted pipe in a construction pig.
Air comes out too fast and the pig becomes a projectile.
|
|
|
|
12. When laying fill line off R.O.W. check for
hazards not identified on R.O.W. assessments, particularly overhead
powerlines.
|
|
|
|
13. When a test is completed, before cutting heads
off pipe, check, then double check pressure is off the line - do not
take it for granted.
|
|
|
|
14. When air testing, two inch iron is very hot -
caution burns.
|
|
|
|
15. When grinding test heads in the ditch, set the
head on solid support rather than having it suspended, in case of
equipment failure.
|
|
|
|
16. Use Contractor provided LiftAll Tuflex Polyester
Roundslings, when lifting test heads. Don’t use owner slings - often
they are undersized.
|
|
|
|
17. Expect a pipe wrench to slip - avoid banged up
knuckles.
|
|
|
|
18. Check out test heads – Do they have a
grating/platform the workers can stand on when hammering wrenches on
flange bolts?
|
|
|
|

socket size for nut size or wrench will jump out of
control

In confined space get
good posture prior to swinging sledge hammer or you will miss
the wrench and hit fingers
|

Slow down – don’t get in
a hurry – severely crushed fingers are common

20. Skidding up end of pipe in ditch - watch for
others when throwing skids down in ditch - watch toes and shins when
building cradle.

|
21. When cutting a section or before booms snug up
on belts or pick up any section - Straight wall or
sidebend:

and away from the pipe

Get workers out from
between pipe and vehicles or equipment. When making a cut,
park the welding rig well away from the cut and roll out the
hoses. When hooking onto a section, side boom operators are
not to snug up belts until foreman gives the signal – even a
slight release of pressure on the skid piles can initiate
movement of the entire section – this also applies to sections
on skid piles above ditch. The section, as it is lifted and
pressure comes off the skid piles can be under tension and
slam against the ditch wall crushing anyone in the ditch. When
making a cut, it can spring violently to one side striking an
employee or catapulting him toward any nearby object such as a
welding rig. This is particularly common when cutting fill
line on the test crew.

22. When uncoupling water hoses, get a secure grip
on the coupling - on stiff hoses the coupling can spring back and
hit you in the face.
|
|
|
|
23. Don't walk under a side boom under load - walk
around the back of the side boom - equipment can fail.
|
|
|
|
24. Wear safety glasses when hitting clamps, hammer
wrench or cable cutter with a sledge hammer.
|
|
|
|
25. Before using Methanol - crew must attend a site
specific WHMIS training which includes a review of the Methanol MSDS
sheet.

26. Welding on windy days - hot slag burns to eyes
common - use wind
brake.
